In order to engage in the certification process, you must currently be working with a Yearly Training Plan (YTP) and submit it for review to ensure the plan is at an appropriate level of development to effectively participate in the performance planning training module and/or proceed directly to evaluation.You may submit your plan in whichever format you feel most comfortable with. A copy of the template used in the Performance Planning training module is available for download.

If you are applying to participate in the cohorted program, please include a copy of the plan you are using for the current season. It will serve as a baseline upon for you build upon as complete the training throughout the cohort.

If you are submitting for the purposes of going direct to evaluation, this is the plan you will be including in your portfolio and that will be accessed as part of the certification process.
